DESCRIPTION:
Ontario Shores is a 340-bed, academically-affiliated, behavioural health facility in Whitby, Canada. Ontario Shores is a Recovery facility, thus self determination, empowerment, and the instillation of hope are a critical part of our mission, in serving forensic patients. Further, the Canadian landscape has seen numerous class action lawsuits regarding use of control measures, both on a federal level, and provincially. Accordingly, we felt it critical to become as effective as possible in reducing control interventions. Ontario Shores is also a HIMMS “triple 7” facility, the first behavioural health facility to receive this designation reflective of advanced use of data and analytics, in support of this aim.
Ontario Shores began its journey in reducing restraint and seclusion close to 10 years ago, recognizing that control measures are not therapeutic to patients, and increase risk of harm to both patients and staff. This may contribute to post traumatic symptom burden, which is overrepresented in persons with serious mental illness, and in point-of-care nursing staff.
Ontario Shores has proceeded in a sequential fashion, employing evidence-based interventions. We have layered, over time, trauma informed care, the Six Core Strategies, SAFEWARDS, and the use of the DASA-IV with the Aggression Prevention Protocol (APP). As a result, we have reduced control measures more than 90%; in 2016, Ontario Shores typically logged over 3000 hours of restraint or seclusion per month. Ontario Shores now logs approximately 200 hours of restraint or seclusion per month. This reduction has also led to a decrease in violent events involving staff, and significant reduction in lost time injury (LTI), resulting in considerable cost savings to the organization.
